We offer easy solutions for making a referral:

Home Health care services must be ordered by your doctor.

​

  • If you are in the hospital, talk with your discharge planner or case manager for help with getting the referral from the doctor. The hospital will send us the referral on your behalf. 

​

  • If you are not in a hospital, talk with your doctor about getting a referral. You can call our office if you need help talking with your doctor. Your doctor's office will send us the referral on your behalf.
